Enrollment Agreement

Report of Health Examination for School Entry

The Health Examination Report must be completed before student starts kindergarten or first grade. This ensures the health and well-being of students as begin their educational journey.

Oral Health Assessment Form

The oral health assessment form must be completed by a dentist or a waiver signed before school entry. Please submit the required documentation on time to ensure your child's enrollment. Thank you!
